About Fallon Community Health Plan
Fallon Community Health Plan (FCHP) is one of America's foremost managed care plans, with more than 180,000 members throughout central and eastern Massachusetts. Founded 26 years ago, the health plan's network includes more than 40 hospitals and thousands of private practice physicians. The health plan offers a choice of targeted products, including Health Maintenance Orgranizations (HMO), preferred provider organization (PPO), and Medicare Advantage plans.

HMO (Health Maintenance Organization)
Members of the Fallon Community Health Plan HMOs receive coverage for routine and preventive care services with little or no paperwork such as bills or claim forms. Care is typically covered in full after a small co-payment or a plan deductible. The HMO plans are available to the self-employed and small business owner.

PPO (Preferred Provider Organization)
As part of providing a wide range of plan options Fallon Community Health Plan now offers Preferred Care PPO to employer groups. PPOs offer coverage with the added advantage of gaining services without a gatekeeper. Premium savings are available due to in and out of network deductibles and co-payments.

HSA (Health Savings Account)
The Choice Plan is a high deductible HMO plan that offers access to medical care without a gatekeeper. To supplement this coverage and pay part of the deductible the plan can include a HSA into which pre-tax contributions can be made to pay for routine medical expenses. The HSA account is available forever and earns interest until it is used. This rollover feature can provide a cushion for medical expenses in succeeding
years.

RETIREE HEALTH PLANS
Two plans are available to group retirees and working employees that have attained age 65.

FALLON SENIOR PLAN HMO
This is a Medicare Advantage Plan for people who are entitled to Medicare or are already enrolled in Medicare. Members of THP Medicare Preferred HMO have access to over 2,600 physicians including 270 Primary Care. The plan includes unlimited prescription drug coverage with only small-tiered co-payments.
FALLON SENIOR PLAN PPO
This is a Medicare Advantage Plan for people who are entitled to Medicare or are already enrolled in Medicare. Members of THP Medicare Preferred PPO have access to over 4,800 physicians including 580 Primary Care. The plan includes unlimited prescription drug coverage with only small-tiered co-payments. It also allows access to any Medicare participating doctor in the United States. Higher co-payments are charged for non-contracted providers.

Emergency Care
Members are always covered for care in a serious medical emergency. Members should receive treatment at the nearest emergency facility, call 911 or another local emergency number.

Worldwide Coverage
If traveling and in need of immediate care, members go to the nearest medical facility. Fallon Community Health Plan covers any unexpected or unforeseen care needed when traveling outside of the service area.
